Attached please find two versions of the Coach Application Form for the 2027 season for those coaches who are interested in applying for a Rep or Select Head Coach position.  (Assistant Coaches are selected by each Head Coach, so no application form is required to be an Assistant Coach.)

 

If you have coached in a Rep/Select capacity with the CVBSA for 3 YEARS OR MORE including the 2026 season, please complete the SHORT VERSION of the form.

 

If you are new to coaching, or have coached in a Rep/Select capacity with the CVBSA for LESS THAN 3 YEARS including the 2026 season, please complete the LONG VERSION of the form.
